I've only tried it once. It was an early 80s silver P-Bass logo. I had no instructions and I attempted to apply it like any other decal. It went on great and I was happy... but as it dried it started lifting at the edges and curling up. I tried to re-wet it and smooth it back down and it broke into pieces.
For one thing, it was really old (NOS) to begin with. For another, I later read that it takes a special adhesive solution, not just plain water.
So I really can't help other than to say that it takes some kind of special glue or procedure that I don't have or know.
Sorry!

I know they were always under the finish. I don't know how quickly the neck was finished after they were applied, but I assume durability and adhesion are big factors with these.
Edit: Let us know what you find out!

Next time you do decals get some MicroSol and MicroSet. These two products are for decals in general, but for decals like these they are really worth their weight in gold, especially if you've got a vintage decal.
