A one off as far as I know. The seller didn't remember the name of the builder who put it together.

The kinko print job turned out pretty well. The amp isn't here yet, but I figured if any of you out there don't do custom faceplates or paint work for your controls and have a custom amp you would like nicer labels for, this stuff might help you. I can give the cost involved with the options and pictures of how they look and such so you can have some better options than taping a piece of paper to your head or using sticky notes to label your controls.
The kinkos print job was $14ish. It is printed on clear vinyl. (I can't tell for certain it is yet because the backing is white so they look white I am guessing.) I paid for an 8.5 x 11" decal and cut them out using their paper cutter in the store. It did the job great.

I think they printed pretty well. The orange I got printing isn't exactly what I had on my monitor, but that often happens for me when it comes to screen vs print results. They printed clearly and should work well if I end up using them. I will wait for the transfer ones I ordered to arrive first though, because they won't have any border if they look good vs these have that clear square/rectangle look when applied unless I trim them more closely.
The transfer vinyl ones were only $8 but they haven't come yet so I don't know how they look just yet. I got a set in orange (no outlines) and black to have options depending on what I do with the faceplate color.

It came in and sounds great, but there is a problem. The presence control and boost don't seem to do anything. I turn the knobs full range back and forth and no sound difference.

Could it be tubes? There are four big ones and two little ones. Not sure the type though.

I tried swapping the smaller tubes with some ECC83's I had. (One in it was an ECC83. The other one didn't have any label I could find.) Still has the issue....

Sounds like since the seller had shipping protection Reverb might cover the repair cost instead of paying out the replacement value. That would be pretty sweet because I love how it sounds. I just am worried because if two parts aren't working, it could lead to something more catastrophic later. My transfer stickers came in. They look great, but they are quite large. Here is picture. You can see them next to my Kinkos ones below.

Reverb needs me to get an estimate. I sent some emails. I got one "I can't help you" email back so far from Alvarez Tube Amp Repair. Hopefully one of the others I emailed will be able to take a look and get it back in shape.

The inputs I am guessing are high and low inputs. How can I tell which is high vs low? The one on the top seems to have a resistor connected to it. Might that be the high input? I don't think the lower one has a resistor directly connected to it.

Ah I found this.
That should mean the input with the resistor is the high input one. (The upper one.) Solderhaus is going to work on it. I will ask them to confirm.You're right on track. Amps with dual input jacks per channel offer you a high and a low gain input. It helps to tame down humbuckers to plug them into the low input gain. But, there is no single goes here, hummer goes there rule. If it sounds good, then that's all that matters.
If you look at the insides, you'll see a 1 Meg resistor attached between two of the three lugs on the high jack.

I understand it (on the Matamp anyway) to be a way to switch between singles and humbuckers without adjusting master volume.
To be fair, I usually just plug into whichever and adjust volume to suit anyway.

Turns out the amp is fine. The boost and presence only show up sound wise if the amp is turned up louder than I turn it up. Here is the details from Solderhaus.
Got your amp on the bench as soon as I got it back to the shop.
There is not one thing wrong with it.
One pwr tube reads 29mA and the other 3 idle at 35. Not a big deal.
Weird layout but works perfect.
BOOST control works in conjunction with the PRESENCE control.
The effect of these controls is reliant on the output signal level.
When the VOLUME control is way down, there’s not a lot of negative
feedback.
It’s a “post-phase-inverter” master volume.
The BOOST control is NOT a gain boost.
